I have seen a few neverending projects in my time (a moment while I give thanks for being retired) and I have to say that in most cases the main problem was as you note in item 2; the insistance of the powers that be that they can achieve two or three times as much with no change in server configeration. I once had a senior manager suggest that we 'just turn it up'.
